Dear Ms. (XX):
It was a pleasure to see (XX) for a followup visit. Results from his blood tests showed cherry +0.36, hazelnut +2.32, but almond, walnut and soybean were all undetectable.
Results of his blood tests and skin tests for hazelnut and soy gave conflicting results.
I think it is reasonable for (XX) to go ahead and eat almonds and walnuts since skin tests and RAST tests are both negative, but he should continue to avoid any of the fruits that cause oral symptoms as well as hazelnuts and soy protein.
I eventually think that the soy protein could be added back into his diet.
I look forward to seeing him back in a year.

Dear (XX):
It was a pleasure to see you recently in the allergy department. Results from your recent blood tests showed a mildly elevated C-reactive protein of 9 (normal less than 5). This is a nonspecific measurement of inflammation going on in the body such as an infection or some other type of inflammation. That might have something to do with your hives and swelling.
Your remaining tests that we did showed no significant abnormalities, including your blood count, C4 complement, antithyroid antibodies and antinuclear antibody.
Hopefully, the switch to cetirizine will be effective. See us back in two weeks. We will continue to monitor the CRP intermittently.

Results of (XX)’s blood tests showed an IgG of 607 (increased from 5.59), IgA 94, and IgM 78. All these levels had increased appropriately. The IgG is within the normal range.
Ragweed pollen and white pine pollen were both undetectable.
There is no direct evidence that she is sensitive to any pollen that might be causing her symptoms based on these tests.
I look forward to seeing (XX) at her next visit.

It was a pleasure to see you for an allergy evaluation. This is a brief note to let you know that allergy testing on your blood for cod, salmon, tuna, swordfish, catfish, egg white and tilapia was undetectable.
I do not believe that you had an allergic reaction to the fish, but, in fact, most likely had a scombroid poisoning, sometimes known as histamine poisoning, caused presumably by bad fish.
Occasionally, certain kinds of fish can become spoiled producing a lot of extra histamines that when ingested can cause what appears to be an allergic reaction.
There is no evidence any longer that you are allergic to egg either. Unless you have any further reactions, I do not think it is necessary for you to keep the appointment of (XX)th. If you want to cancel the appointment, you can and return only if you have any further problems.

It was a pleasure to see (XX) for followup visit. Results from his recent blood tests show walnut 2.48 (previously less than 0.35), shrimp 6.58 (1.69), pecan 2.02 (less than 0.35), egg white 1.52 (previously 0.96), milk 9.64 (7.17), peanut 4.92 (0.9), hazelnut 69.6 (31.7), and cashew, Brazil nut, almond, salmon, pistachio, pine nut and codfish were undetectable, less than 0.35.
Unfortunately, some of the test results such as peanut, egg and milk have increased. Although the allergies have improved significantly from before, he still has significant positives and should avoid all foods that he is allergic to. We will continue to monitor him on a regular basis. It also appears that he has developed sensitivity to pecan and walnut since he was last tested.
There was a very mild allergy to dust mites that we detected at 0.40 and tests have shown sensitivity to dust mites but at a mild level in the past as well. I recommend that you follow dust precautions in the bedroom, including zippered encasings for the mattress and pillow, frequent washing of the bedding and eliminating as much clutter as possible. The bedroom should be vacuumed frequently as well.

It was a pleasure to see (XX) for a followup visit. Blood tests confirm sensitivity to nuts and peanut. Peanut was quite high at 39 with lower levels to cashew 2.85, hazelnut 1.18, pistachio 4.4, walnut 0.39; but almond, Brazil nut, pecan and pine nut were undetectable, less than 0.35.
Tests for fish were undetectable for cod, salmon, tuna and swordfish.
Based on these results, it is unlikely that he is still significantly allergic to fish, but I would like him to be skin tested to individual fish before we allow him to ingest them again.
Shellfish showed excellent results with the exception of a very slight reaction to shrimp at 0.38. Lobster, crab, scallop, blue mussel, clam and oyster were undetectable, less than 0.35. Again, I would like him to be skin tested for the shellfish before we allow him to ingest them.
Egg white was undetectable. This is an excellent sign that his egg allergy is in the process of disappearing.
He should strictly avoid peanuts and tree nuts. If you would like to pursue the fish and shellfish question further, we can arrange for him to be tested before his next visit, which is a followup visit in one year. We can do this any time during the course of the next 12 months. He would need to be off antihistamines for about a week ahead of time. Let me know if you have any questions.
