PROCEDURE PERFORMED: Bilateral carotid ultrasound duplex evaluation.

INDICATION: Transient ischemic attack.
FINDINGS:
Right carotid system: A 2-dimensional analysis reveals mild atherosclerotic disease identified in the right internal carotid artery.
Spectral analysis reveals peak systolic velocities of 1.1, 0.88, 0.92 m/sec in the common carotid artery, internal carotid artery and external carotid artery respectively. Antegrade right vertebral artery flow.
Left carotid system: A 2-dimensional analysis reveals mild atherosclerotic disease identified in the left internal carotid artery.
Spectral analysis reveals peak systolic velocities of 1.1, 0.78 and 1.2 m/sec in CCA, ICA and ECA respectively. Antegrade left vertebral artery flow.
IMPRESSION:
1. No hemodynamically significant stenosis.
2. Mild atherosclerotic disease bilaterally in the internal carotid arteries.
3. Antegrade flow in vertebral arteries was seen.

STUDY PERFORMED: Carotid Doppler study.

INDICATION FOR STUDY:
INTERPRETATION:
Right Carotid System: RCCA velocity of 84, RICA velocity of 115, RECA velocity of 407. RICA/CCA ratio was 1.36 with antegrade vertebral flow at 58. This indicates about 40% stenosis of the right internal carotid artery and about 99% stenosis of the right external carotid artery.
Left Carotid System: LCCA velocity of 87, LICA velocity completely occluded on the left, LECA velocity of up to 30%. LICA/CCA ratio, there was antegrade vertebral flow at 102.
CONCLUSIONS: Moderate to severe atherosclerotic plaque seen bilaterally. There is an occluded left internal carotid artery and mild stenosis of the right internal carotid artery with severe right external carotid artery disease. There is antegrade vertebral flow bilaterally.
RECOMMENDATIONS: Yearly followup and aggressive lipid therapy to maintain patency of the right internal carotid artery as much as possible with close monitoring.

INDICATION FOR STUDY:
INTERPRETATION:
Right Carotid System: RCCA velocity of 95, RICA velocity 103, RECA velocity of 200. RICA/CCA ratio of 1.08 with antegrade vertebral flow at 101. This indicates up to 30% stenosis of the right internal carotid artery and 60% of the right external carotid artery.
Left Carotid System: LCCA velocity of 84, LICA velocity of 147, LECA velocity of 156. LICA/CCA ratio of 1.75 indicating up to 59% stenosis of the left internal carotid artery with antegrade vertebral flow at 50.
CONCLUSIONS: Moderate atherosclerotic plaque bilaterally. Mild to moderate stenosis in the left internal carotid artery and in the external carotid arteries bilaterally with antegrade vertebral flow bilaterally.
RECOMMENDATIONS: Aggressive lipid and antiplatelet therapy and recheck every 12 months.

STUDY PERFORMED: Carotid ultrasound study.

INDICATIONS: Evaluation of a (XX)-year-old right-handed female with suspected syncopal episode. Risk factors for cerebrovascular disease include only a history of tobacco abuse.
TECHNICAL DATA: The study consisted of duplex Doppler ultrasonography of the extracranial cerebral vasculature with color augmentation as well as spectral analysis with estimates of average peak systolic and mean end diastolic flow velocities. Technical data as well as the CD are reviewed.
FINDINGS: No significant atheromatous plaque formation is observed. Intimal lining is smooth and without obvious irregularity. Spectral analysis is entirely normal, as are the internal carotid artery/common carotid artery ratios, with the sole exception of minimal elevation of the average peak systolic velocities through the proximal portions of the right CCA and the left ICA, suspected to be due to hyperdynamic flow as no particular stenosis is observed in these regions. Vertebral flows are antegrade bilaterally.
IMPRESSION: This carotid ultrasound study is essentially within the range of normal variation for this patient’s age. No significant atheromatous plaque formation is observed. The intimal lining is smooth and without obvious irregularity.
